This project aims to empower 240 socially isolated residents aged 50 plus to gain better access to primary health-care services and improve their health. Through this a number of groups came together and identified a need to work with local residents to tackle isolation. The project will work with five organisations to provide an integrated and holistic community approach using four key elements; one to one health advocacy; befriending; volunteer driver service; and resident’s self-advocacy group.
