The project will support a minimum of 150 young disabled people aged 14-25 living in Swansea with their transition to adulthood. This will be achieved through a 'one stop shop' weekly drop-in group, support in schools and through a team of transition key workers. The aim is to reduce isolation and increase their well-being by supporting them to make choices and achieve their future ambitions. The project will also seek to increase joined up working between existing services in order to improve access.
