Supporting Young People Transitioning from Children?s to Adult Services in Cambr - Prosiect

The funding will be used to support the Arthur Rank Hospice Charity in Cambridgeshire, Sue Ryder Thorpe Hall Hospice in Peterborough and East Anglia Children's Hospices in Milton to work together to develop a programme to ease the transition from childrens to adult hospice services, through the creation of a joint role of Young Adult Transition Coordinator. The role will work across the three hospices to establish continuity of care and to ensure that young people and their carers feel supported and welcomed to the adult hospices, including creating a programme of social activities designed to meet the needs of the young people.
