The project will deliver activities to address social isolation with young disabled people, aged 14-25 (and their families), or who may have learning difficulties, autism or experience mental health issues. The aim of the project is to increase opportunities available for young people and supporting them to form valuable friendships and taking them through the difficult transition into adulthood.
