Health and Well being for Family Carers - Project

Project overview

This new project will support carers and the people that they care for to help manage long term health conditions and support with end of life care. There will also be a focus on identifying and reaching out to isolated 'hidden carers' who do not access any support, reduce acute hospital admissions as well as providing information on lifestyle changes to ease the role of the carers and provide better managed support for the person they are caring for and their condition.
