Project overview

T Hafan Children's Hospice will provide crucial crisis care and community family support services to those families of life limited children most in need throughout South West and Mid Wales through a �virtual community hospice�. Family Support Practitioners and Complementary therapists will provide support and assistance to alleviate the additional issues caused by the COVID-19 pandemic. Bydd Hosbis Plant T Hafan yn darparu gwasanaethau gofal argyfwng a chymorth teulu cymunedol hanfodol i�r teuluoedd hynny o blant � chyfyngiadau bywyd mwyaf anghenus ledled De Gorllewin a Chanolbarth Cymru trwy �hosbis gymunedol rithiol�. Bydd Ymarferwyr Cymorth i Deuluoedd a therapyddion cyflenwol yn darparu cefnogaeth a chymorth i leddfu'r materion ychwanegol a achosir gan y pandemig COVID-19.
