This four year SIB aims to enable up to 1,190 people at the end of life to achieve their wish of receiving a better quality of care in their home and eventually dying at home, rather than in a hospital, where that is their preference. A single point of access via a telephone co-ordinated hub will improve services and fill existing gaps. The SIB will generate net savings of up to £1.36m to Waltham Forest CCG. £1.5m social investment is expected to generate up to 8.2% annual returns.
