Project overview

The project will facilitate a programme of 'Community Prescription' to physical activity designed to prevent ill health and improve the management of chronic conditions. This will involve working with the community and health organisations to promote General Practitioners to refer and accept self referrals to the programme. The project aims to get 5,400 beneficiaries accessing the physical activity sessions in local natural environments, whilst using innovative technology, to monitor beneficiary progress.
