Help using our PDF forms
You should always use Adobe Acrobat Reader to complete our PDF forms.
Using other software can lead to your form not saving properly, or not being readable when you send it to us.

Make sure you’ve got the latest version of Adobe Acrobat Reader installed. It’s free to download and works on all common devices.
How to complete your PDF form
Step 1: Download the form
And save it to your device.
Step 2: Open the form using Adobe Acrobat Reader
First open Adobe Acrobat Reader on your device. Then open the PDF form using Adobe Acrobat Reader.
Do this each time you want to work on the form.
If you open the file another way it could open in other software, like your internet browser, which may not save your work.
Step 3: Type into the form, or use plain text
If you need to copy and paste your text, first turn it into ‘plain text’ to remove formatting. You can use an online ‘plain text converter’, or copy your formatted text into a plain text only app like Microsoft Notepad first.
Step 4: Save your form
Make sure to save your form regularly as you go, and when you’re finished.

Use clear language that is easy for us to understand. But do not worry if you’re not able to format your answers as well as you’d like to.
