Reaching Out - Young Adult Carers Project 16-35 - Project

Project overview

The project aims to provide support and better access to services for young adult carers as they transition into adult services as well as parent carers who fall between children and adult social care service. Support will be provided through home visits, activities in community venues, GP surgeries, cafes and anywhere else a carer may feel comfortable or able to talk. This will be helpful in reducing barriers in rural locations where transport is limited and will enable access for all.
